NCLEX Application Process:

Go to www.nursing.ohio.gov and click on the Forms and Application heading located on the left side of the page. Scroll down until you find the heading Forms for unlicensed Ohio Nurse Applicants (NCLEX), below this heading is the heading Examination Application Packet, click on this heading and print this packet. This packet explains the whole process for applying for the NCLEX ie. how to register for the NCLEX on-line at the Pearson Vue website, how to complete the paper application for the Board of Nursing which includes the 2x2 picture, application fee, notarization, where to send the paper application, and background check information along with how to find a Webcheck location. Also, Terra’s school code is 20-456, which will be needed for both the on-line and paper application.
