NCLEX and Ohio Board of Nursing Registration

Here are the forms that I used to register for the Ohio Board of Nursing License. You will find all of the information and instructions in the below documents. To print the forms click on the ipaper icon at the top of each entry and select print. I would also strongly suggest reading the 2009 NCLEX Examination Candidate Bulletin from Person Vue. This bulletin describes the entire NCLEX registration and test taking procedure. The link for this document is located on the right hand side of the blog under NCLEX-RN Resources.
